Shuttle Columbia Accident Weekly Briefing Officials spoke to reporters about the progress of an investigation into the loss of the Space Shuttle Columbia. They said that they had reached a stage in the investigation at which they would concentrate on coolant foam striking protective tiles upon liftoff as the working hypothesis for the ultimate cause of the accident.
