Chief of Naval Operations Admiral John Richardson held a briefing to outline the investigation findings involving the USS Fitzgerald and USS John S. McCain collisions. Admiral Richardson took full responsibility for the incidents saying, “I own this and I won’t dodge from that ownership.” He said the deadly sea collisions were “preventable” and caused by a series of failures including declining safety standards, officer fatigue, a loss of situational awareness, and insufficient knowledge of the ship steering system. In August 2017, the USS John S. McCain collided with an oil tanker off the coast of Singapore, killing 10 sailors. In addition, seven sailors died in June 2017 when the USS Fitzgerald collided with a cargo ship in the Sea of Japan. close
