A new Pentagon investigative report cites a series of failures, shortage of personnel, and inadequate training for the deaths of four U.S. special operations services members in an ambush in Tongo Togo in Niger on October 4, 2017. None of the four were ever captured according to the report and all are expected to be recommended for the Medal of Valor award. No one in the chain of command has yet been held responsible for the ambush according to AFRICOM Commander General Thomas Waldhauser and the investigating officer Major General Roger Cloutier. close
